A Diagnosis and Treatment Plan

Choose an individual on whom to complete a full alcohol and drug assessment and write a report with a diagnosis and treatment plan. This individual can be someone you know, someone you met at a meeting, or a fictional character with alcohol or drug problems from a book or movie. If you choose a real-life person, use a pseudonym (fake name) for him/her in this paper. Such as John Doe or Jane Doe. It must be clear that you are using a fake name, or you risk receiving a 0 for the assignment.CREATE THE DETAILS based on what you know—you are not interviewing someone for this assignment. The only criteria are that the person/character you choose must have a history of a substance use disorder. The treatment plan must be applicable for the time of substance use being reported.The paper must include pertinent information in the case history to justify the treatment plan.  Also, consider relapse issues when designing the treatment plan and this paper must include a detailed assessment and thorough treatment plan recommendationsCreate the relevant details that would fulfill the assignment requirements (for example, CAGE results).  The paper must follow the current APA format. It should be approximately 13 pages, not including the title and reference pages. Also, 7 scholarly sources (books, journal articles), as well as two Bible references, must be included in the paper.The header and Footer must be listed as THE FINAL ASSESSMENT PROJECT  on all pages except the Cover Page of the document.
